Coach and Substitute Paperwork
Upon notification that you have been recommended for hire, you are required to complete a hiring packet. These materials can be picked up at the Central Office reception desk or can be downloaded below. Please bring the completed forms with you to your hiring appointment.
Take the KDE Medical Examination of School Employees Form with you to your doctor appointment. You must receive three tests: health exam, TB risk assessment, and drug test. It is your preference where you get your health exam and TB risk assessment. If you choose to make an appointment at Baptist Health (270-825-7351), indicate that you are a new employee for the school district. In addition, you must also participate in a drug test. The drug test MUST be completed at Baptist Health ($40.00); they will fax the results of the drug test to HR. The district does not reimburse you for these tests.
Bring with you to your appointment: an original college transcript showing 64 hours or more (high school coaches) or a copy of your high school diploma (middle school coaches) and a $20 money order for a criminal background check. The district does not reimburse you for this check. Also, bring your social security card and your driver's license.
Apply for the Substitute Teacher position online under Job Opportunities. Mandatory online training must be completed before the candidate will be invited for an interview.
Substitute Classified (CIA, Cook/Baker, Custodian)
- If you are applying for a Substitute CIA position, you must also provide a copy of high school diploma or GED as required by Kentucky law AND a copy of results of paraeducator test.
