2021 Sports Physical Flyer

Good morning,

I have attached the flyer with information on the athletic physicals for this year. Will you please share with all coaches? Due to our continued COVID restrictions we are limited by the number of people in the building thus we are dividing the athletes by alphabet. We ask that athletes come at their specific times.  Please encourage athletes to have their physical forms completed prior to arriving at Sports Medicine.  All athletes will also need to wear a mask or face covering.
